Medicare Open Enrollment info session via telephone
Please pass the word among your homebound clients over the age of 50 who are socially and physically isolated in the community about upcoming Tele-Learning Sessions. Attached is a flyer to post and how to make a good referral to the program.

Family Eldercare's Lifetime Connections Without Walls program (LCWW) offers socialization, brain fitness activities and educational presentations to homebound older adults who are socially or physically isolated in the community. Older adults join in from their homes using their telephone and dialing a toll free phone number and an access code. Older adults also have the opportunity to have a volunteer facilitator connect them to the group call. Participants join in from all over the state of Texas. Volunteers can join in from home or work or come to our office to facilitate the call. The program goal is to alleviate social isolation, and loneliness among participants by providing opportunities for meaningful conversations, friendships and peer support and also empower them. CALL TO PRE-REGISTER!512-220-7076. Once registered, you will receive the toll free number to join the call. For clients who qualify based on income, there is no fee to join the call. Registration fees applicable depending on income.
50+ - Socially or Physically isolated in the community
Ability to have a conversation over the phone
Participant should be comfortable been on the phone and in a group situation.
Memory loss and hearing impaired seniors are not a good fit for this program.
Access to a telephone
Clients need to be registered to join the sessions.
